Service Delivery Framework
Thank you for your participation in the Service Line Subcouncil Group. In April 2015, a workgroup of the Board outlined a framework of regional services from primary care and diagnostics facilities, to specialty medical pavilions, a freestanding medical facility at Queenstown, ambulatory hospitals in Chestertown and Cambridge, and a Regional Medical Center in Easton. Using the framework as a guide, please submit your group's recommendations below that supports the proposed framework OR explain why the framework will not support the delivery of health care consistent with our mission and vision for Shore Regional Health.
Guidelines for Use of Group Recommendation Form
This form is to be used as a compilation of your Service Line Subcouncil recommendations relative to the framework. The first page will provide the Strategic Planning group with your overall recommendations based upon your diligent analysis of the data, group discussion, identified needs and potential issues. The subsequent pages will provide the details behind each of your recommendations.
There is room on this survey for eight (8) recommendations with detailed explanations. If you have less than eight recommendations, skip over the remaining survey pages by clicking "Next" and click the "Submit" button on the last page. If you have more than eight recommendations, you may start a new survey and in the "Sub-Council Members" field on the first page - simply enter "Continuation of first survey".
Make sure you click the "Submit" button on the last page when the form is complete. The form does have an autosave feature so you may leave the form unfinished and return to it using the same computer if needed. If for some reason you need to clear your entries, there is a "clear form" button on the last page. You will receive an email notificiation once your submit the form with a link to use if you need to go back and edit your submission.
A successful report includes:
· Synthesis of data in the decision making process. Recommendations should either be supported by data, or in case of a recommendation that seem to run contrary to data, the rationale behind the recommendation.
· Constructive ideas with specific observations that illustrate the changes being suggested.
· Fair and candid appraisal of the current environment. Recommendations are meant to reflect where the organization is currently situated and where we ideally need to be.
· Fair representation of group outcome. Where there is a dissenting view, that view should be included in the feedback so that all voices are represented.
