Application for Matching Funds
Part 1
Information about Fundraising Activities
Match funding, online giving and other support is awarded to qualifying fundraising efforts based on an application process. Applications are reviewed monthly by a committee with medical, legal, social service and financial experience. Funding preference is extended to efforts that invite broad community support by offering many ways to volunteer and give for the sole purpose of helping a family burdened by substantial expenses due to a catastrophic medical condition.
Application Deadline: Applications are due by the first and third Monday of each month for a fundraising event schedule in a following month - at least 30 days prior event and before information/flyer is publicy shared.
Part 1 of the application includes information about fundraising activities to be completed by the lead volunteer of the fundraising effort.

You must have the following information available to submit the application online.
______1: Volunteer Information (Name and contact information for lead and 2 other key volunteers)
______2: Benefit Fund Information (attach copy of signature card validating account signers)
______3: Fundraising Activities (Event location, address & date)
______4: Lend A Hand Referral Source
______5: Nonprofit Endorsement Confirmation (Form provided for lead volunteer to forward to nonprofit)
______6: Lead Volunteer Signature
