Please complete all the information on the following pages. Missing information may delay the adoption screening process. Completion of this application form does not guarantee that Mountaineers For Mutts (M4M) will place one of our pets in your care. Completion of this questionnaire authorizes M4M to verify any information contained herein, including verification of medical records of any present or prior pets treated by the veterinarians listed. Completion of this application is an indication that you have read, understand and agree to abide by our Animal Adoption Agreement and that the information contained within this application is accurate and not misleading in any way. All adoptions are at the sole discretion of M4M.
Please give careful consideration to adopting an animal. Pets are not toys or short-term commitments.

Make sure your lifestyle allows the time, patience and expense this animal will need over the years.
All pets will be spayed/neutered, vaccinated, treated for parasites and microchipped prior to adoption. Approved adopters are expected to travel to Morgantown WV to pick up their pets. Adopters in northeastern states, we may be able to work with a local rescue near you to facilitate an adoption.

Please Note: If you live in MA or NH, you are required by law to put your newly adopted dog into a state-licensed quarantine facility upon arrival. Please look into that before completing this application.

Please Note: For adopters outside of our local area (Morgantown WV), we must find a back-up rescue near you in order to be able to consider you for adoption. We work with a network of rescues in multiple states and will check with our network as part of the application process. If for some reason you are not able to keep our adopted dog, we will have a rescue on standby to retrieve the dog for us. Do not take any of our dogs to a shelter near you. We will retrieve them in the event of any failed adoptions. If we are not able to find a back-up rescue near you, then we're sorry but we will not be able to adopt to you. We only do adoptions where there can be a safety net in place for the dog. Thanks for understanding.
