  • Part 3a: COVID-19 Health Assessment
  • Have you or anyone else in your home experienced a fever of 100.4 degrees or greater, a new cough, new loss of taste or smell, or shortness of breath within the past 10 days?*
  • In the past 10 days, have you or anyone else in your home tested positive for COVID-19 using a test that tested saliva or used a nose or throat swab (not a blood test)?*
  • To the best of your knowledge, in the past 14 days, have you or anyone else in your home been in close contact (within 6 feet for at least 10 minutes) with anyone while they had COVID-19?*

    • 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or
    • 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine
  • Including yourself, have all individuals in your household been fully vaccinated?*
