Part A: Code of Conduct
Ø Abusive language towards a staff member, volunteer or another participant.
Ø Possession or use of alcoholic beverages or illegal drugs on Murphys Educational Center (MEC) Inc. property or reporting to the program while under the influence of drugs or alcohol.
Ø Bringing onto MEC’s property dangerous or unauthorized materials such as explosives, firearms, weapons or other similar items.
Ø Discourtesy or rudeness to a fellow participant, staff member or volunteer.
Ø Verbal, physical or visual harassment of another participant, staff member or volunteer.
Ø Actual or threatened violence toward any individual or group.
Ø Conduct endangering the life, safety, health or well-being of others.
Ø Failure to follow any agency policy or procedure.
Ø Bullying or taking unfair advantage of any participant.
Ø Failing to cooperate with an adult supervisor/leader/mentor.
I have read and I understand the MEC’s Code of Conduct. I agree to abide by the rules described above and understand that I may be removed as a participant if I violate any of these rules.

Part B: Releases
1. Liability Release
In consideration for my voluntary participation, I hereby agree to release, discharge, waive, agree not to sue and to hold harmless Murphys Educational Center Inc., and its directors, officers, representatives, employees, agents and volunteers "(Releasees") from any and all claims or liability for injury, loss, damage, obligation, expense or penalty, including attorneys’ fees, and that I may sustain in connection with my participation in any activity on behalf of Murphys Educational Center including travel and lodging associated therewith, form any cause whatsoever whether caused by the negligence or carelessness of the Releases or otherwise.
2. Assumption Of Risk
I assume all risk, known or unknown, foreseeable and unforeseeable, in any way connected with my participation in any of MEC activities or projects. I accept personal responsibility for any liability, injury, loss or damage resulting from my participation.
3. Indemnification
I agree to indemnify and hold harmless Releasees from any and all claims for any liability, injury, loss, damages, or expense, including attorneys’ fees (including the cost of defending any claim I may make, or that may be made on my behalf, that is released or waived by this agreement), in any way connected with or arising out of my participation.
4. Medical Treatment Permission and Agreement to Pay Medical Expenses
I grant Releasees permission to authorize routine or emergency diagnostic procedures and/or routine or emergency medical treatment, if deemed necessary in medical judgment. I assume responsibility of all resulting bills and cost, if any, associated with said medical procedures or treatment. I understand that Releasees assume no responsibility for any injury or damage which might arise out of or in connection with such authorized emergency medical treatment. I agree to indemnify and hold harmless Releasees from any and all claims, demands, actions, rights of action or judgments I may have arising from said medical procedures or treatments. Should it become necessary for me to return home due to medical reasons, I hereby assume all transportation costs I may incur.
5. Photography Release.
I grant the Releasees permission to be photographed, recorded and/or videotaped and to allow this material as well as any information provided on my application, including but not limited to my name and school affiliation, to be displayed or used for publicity or in promotional materials for MEC. I understand that I will not (a) receive any compensation for the use of my name, voice, image or likenesses; (b) have the opportunity to inspect or approve any photograph or recording. I understand the determination of what is promotional rest solely with MEC.
Part C: Additional Terms
6. Binding Effect upon Successors.
This Release shall be binding upon my relatives, personal representatives, heir, beneficiaries, next of kin and assigns.
7. Severability.
If any term or provision of this Release shall be held illegal, unenforceable or in conflict with any law governing this Release, the remaining portions of this Release shall not be affected thereby and shall remain in force and effect. This Release cannot be changed or altered by any person without the full written consent of MEC.
8. Applicable Law and Venue.
I hereby further agree that this Release shall be construed in accordance with the laws of the State of Michigan. I further agree that the venue for any legal proceeding shall be in Michigan if found necessary.
9. Age/Enrollment Status.
All participants must be 12 years of age older with parents’ written consent. Proof of age may be requested to and/or at on-site registration.
