I authorize Rev. William Marcus McFarland III to charge the credit card indicated in this authorization form according to the terms outlined on this form. This payment authorization is for the goods/services described above, for the amount indicated above only, and is valid for one time use only, unless the contract has been violated by the signed party. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form.
By checking the agree box, I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
Terms and Conditions
By electronically signing your name, you agree to the below "Terms and Conditions" in its entirety. None of the items listed may be disputed in any way.
MARRIAGE LICENSE:
Client understands that it is the client's responsibility to acquire a valid marriage license in the state where the wedding ceremony will take place and the marriage license must be on the premises when the ceremony is performed. Client understands that failure to have these legal documents means that the wedding officiant cannot legally perform the ceremony and is under no obligation to do so. The officiant may at his/her discretion perform a symbolic ceremony, which will have no legal merit, and perform another ceremony in private for the couple once the legal documents are presented, for an additional fee. Client will consult with the local court or city clerk to find the exact requirements for the client's area.
CANCELLATION and REFUND:
The agreement of the officiant to perform the ceremony is subject to proven detention by accidents, riots, strikes, epidemics, acts of God, or any other legitimate conditions beyond their control. If such circumstances arise, all reasonable efforts will be made by the officiant to find a replacement. Should the officiant be unable to secure a replacement to perform the ceremony, client shall receive a full refund. Client agrees that in all circumstances, officiant liability shall be exclusively limited to an amount equal to the performance fee and that officiant shall not be liable for indirect or consequential damages arising from any breach of contract.
All deposits are nonrefundable unless the officiant cancels the wedding.
ADDITIONAL TERMS:
If the ceremony includes a sand ceremony, unity candle, or any other special feature, the client is responsible for furnishing all equipment needed. If the client requests that any additional items are to be furnished by the officiant, the client will be billed for the additional cost of products, plus resources used to obtain them.
If the client wishes to have the officiant attend the rehearsal, it needs to be explicitly added to the package, and an additional fee will apply.
LATE FEE:
Typically, the officiant will arrive at the wedding location about 30 minutes before the ceremony time listed on the "Save the Date" form/contract by the client. Client understands that they must adhere to the times that the client listed on the "Save the Date" form/contract. The officiant does understand that weddings do not always go as planned and will accommodate for weddings that start later than scheduled, as time permits. Officiant will ONLY allow for thirty (30) minutes of time to give for unexpected delays. Any weddings starting more than thirty (30) minutes late will be charged an ADDITIONAL two hundred dollars ($200.00) with the credit card on file.
