Completion of this form is required to apply for participation in the Student Mentoring Program. Once all fields are complete, please click the SUBMIT button at the buttom of the form. Items marked with an asterisk are required. You will receive an email confirming your application and will be contacted by phone or email regarding your application. To be fully considered for participation in the program for the next academic year (September - May), students must submit an online application by September 16. The Mentor-Student matching process will be conducted each summer and matches will be announced in August/September of each year.
